Getting Ready to Go Home - Plan Ahead

MEDICINE:

- Ask questions if you need to know more about the medicine you will be taking at home

- Keep a list of the name and dosages of medicine you take, including vitamins and herbs

- Consider the following questions when you decide where you want to get your discharge prescriptions filled and discuss them with your nurse Will your local pharmacy be open when you get home? Will the pharmacy have the medicines you need?

Certain pain, cancer or transplant medicines are not readily available everywhere. Does the UMH Ambulatory Care Pharmacy accept your drug insurance plan? Call 734.936.8261 to see if your discharge prescriptions are covered by your insurance at the pharmacy.
